Online report requests are for the general public only. If you are from another law enforcement agency, a request on letterhead is required. If you are an attorney, or their investigator, the request must be made either with a Subpoena Duces Tecum or on your letterhead. Private Investigators should make the request on letterhead.

To request a police report, use ONE of these methods:

  • Complete the form below
  • Fill out the Paper Form and return it via ONE of the following methods:
    • Email it to ACSORecords@acgov.org
    • Mail it to 2000 - 150th Avenue San Leandro, CA 94578
    • Fax it to 510-667-3970.

Please fill out the this form if you are seeking the release of protected information. A representative from the Sheriff's Office will review each form submitted to determine whether the information requested may be released pursuant to California and federal law. Completing the form alone will not automatically result in the release of information you are seeking. By filling out this form, you agree to hold harmless the Alameda County Sheriff's Office and its employees, agents, affiliates, attorneys, and representatives, concerning the administering, and enforcing the rules and policies, as related to this Authorization Release Form.

For more information on obtaining a copy of a police or incident report, the Records section is open Monday through Friday 8:30 am to 5:00 pm for most services.

You will be notified by telephone when report is ready for pick-up. Reports must be picked-up between 8:30 a.m. and 4:30 p.m., Monday through Friday, excluding holidays. If desired, the report will be e-mailed or mailed to the person requesting the report by providing a self-addressed, stamped envelope.
