Instructions
- Complete and sign open-ended complaint form - Notarize when appropriate.
- If possible, attach to the open-ended complaint a plot map clearly delineating affected property boundaries.
- Plot maps can be located on Alameda County Open Data Hub (acgov.org). Click on “parcels” and take a screenshot of the address.

- Post clearly visible signs stating that the property or area is closed to the public.
- For electronic submissions, all open-ended complaints shall be notarized by a public notary.
