Open ended complaints are only for Alameda County unincorporated area's

Instructions

1. Complete and sign open-ended complaint form - Notarize when appropriate.

    

2. If possible, attach to the open-ended complaint a plot map clearly delineating affected property boundaries.

    

   1. Plot maps can be located on Alameda County Open Data Hub (acgov.org).  Click on “parcels” and take a screenshot of the address.

       

3. Post clearly visible signs stating that the property or area is closed to the public.

    

4. For electronic submissions, all open-ended complaints shall be notarized by a public notary.

    

   1. Email open-end complaints to acsoopenended@acgov.org. The email is for submissions of this form, not a general complaint email.

       

5. Open-ended complaints submitted by US Mail must be notarized by a public notary.

    

   1. Mail open-end complaints to: Eden Township Substation, 15001 Foothill Boulevard, San Leandro, CA 94578

       

6. In-person submissions shall be made during regular business hours at Eden Township Substation, 15001 Foothill Boulevard, San Leandro, CA 94578. Notarization is NOT required.
